Comparison of three selective media for the recovery of Clostridium difficile from stool specimens

There is a need for effective, economical and readily available selective media for the culture of Clostridium difficile.This study aimed to evaluate the performance of cycloserin-cefoxitin-mannitol agar (in-house CCMA) and cycloserin-cefoxitin-mannitol broth (in-house CCMB), compared to cycloserin-cefoxitin-fructose agar (CCFA, commercial medium for the recovery of C. difficile from stool specimens.The average size of C. difficile colonies on CCMA were bigger compared to those grown on CCFA at 48h of incubation. Bacterial numbers of C. difficile on CCMA was more than on CCFA for 2 stool specimens at 48h (55 colonies on CCMA vs. 9 colonies on CCFA). Of 58 stool samples tested, CCMA and CCMB enhanced C. difficile growth and improved recovery 9 times higher than that on CCFA (18 isolates on CCMA/CCMB vs. 2 isolates on CCFA. Because of the ease of use and lower cost, CCMA was preferable medium to CCMB. Our results suggest that our in house CCMA is sensitive and reduced-cost medium for the recovery of C. difficile from stool specimens.
